VETERANS BROCHURE

During each planning conference our directors will be asking for the deceased’s vital information. Once collected, this information will be used for the completion of the Death Certificate, notification to Social Security and claim forms for Veterans. This form will help you get that information organized.

ARRANGEMENTS PLANNING FORM

The Federal Trade Commission requires that it must contain identifying information, itemized prices for the various goods and services that you sold by funeral homes, and other important consumer disclosures.
